    Aspirin desensitization in patients undergoing percutaneous coronary intervention: A survey of current practice

    Background: Aspirin remains the mainstay of anti-platelet therapy in cardiac patients.However, if a patient is allergic to aspirin and dual anti-platelet therapy is indicated — suchas with percutaneous coronary intervention (PCI), then there is no clear guidance. Onepossibility is aspirin desensitization. A variety of protocols exist for the rapid desensitization ofpatients with aspirin allergy. The aim of this survey was to assess current knowledge andpractice regarding aspirin desensitization in the UK.Methods and results: We conducted a UK wide survey of all UK 116 PCI centers andobtained complete responses from 40 (35.4%) centers. Of these, just 7 (17.5%) centers hadpreviously desensitised patients; 29 (87.9%) centers suggested a lack of a local protocolprevented them from desensitizing, with 10 (30.3%) unsure of how to conduct desensitization.Only 5 (12.5%) centers had a local policy for aspirin desensitization although 25 (64.1%)units had a clinical strategy for dealing with aspirin allergy; the majority (72%) giving higherdoses of thienopyridine class drugs.Conclusions: In the UK, there appears to be no consistent approach to patients with aspirinallergy. Patients undergoing PCI benefit from dual anti-platelet therapy (including aspirin),and aspirin desensitization in those with known allergy may facilitate this. Sustained effortshould be placed on encouraging UK centers to use desensitization as a treatment modalityprior to PCI rather than avoiding aspirin altogether

    The Caledonian face test: A new test of face discrimination

    yesThis study aimed to develop a clinical test of face perception which is applicable to a wide range of patients and can capture normal variability. The Caledonian face test utilises synthetic faces which combine simplicity with sufficient realism to permit individual identification. Face discrimination thresholds (i.e. minimum difference between faces required for accurate discrimination) were determined in an "odd-one-out" task. The difference between faces was controlled by an adaptive QUEST procedure. A broad range of face discrimination sensitivity was determined from a group (N=52) of young adults (mean 5.75%; SD 1.18; range 3.33-8.84%). The test is fast (3-4min), repeatable (test-re-test r2=0.795) and demonstrates a significant inversion effect. The potential to identify impairments of face discrimination was evaluated by testing LM who reported a lifelong difficulty with face perception. While LM's impairment for two established face tests was close to the criterion for significance (Z-scores of -2.20 and -2.27) for the Caledonian face test, her Z-score was -7.26, implying a more than threefold higher sensitivity. The new face test provides a quantifiable and repeatable assessment of face discrimination ability.     Opportunities for Public Aquariums to Increase the Sustainability of the Aquatic Animal Trade

    The global aquatic pet trade encompasses a wide diversity of freshwater and marine organisms. While relying on a continual supply of healthy, vibrant aquatic animals, few sustainability initiatives exist within this sector. Public aquariums overlap this industry by acquiring many of the same species through the same sources. End users are also similar, as many aquarium visitors are home aquarists. Here we posit that this overlap with the pet trade gives aquariums significant opportunity to increase the sustainability of the trade in aquarium fishes and invertebrates. Improving the sustainability ethos and practices of the aquatic pet trade can carry a conservation benefit in terms of less waste, and protection of intact functioning ecosystems, at the same time as maintaining its economic and educational benefits and impacts. The relationship would also move forward the goal of public aquariums to advance aquatic conservation in a broad sense. For example, many public aquariums in North America have been instrumental in working with the seafood industry to enact positive change toward increased sustainability. The actions include being good consumers themselves, providing technical knowledge, and providing educational and outreach opportunities. These same opportunities exist for public aquariums to partner with the ornamental fish trade, which will serve to improve business, create new, more ethical and more dependable sources of aquatic animals for public aquariums, and perhaps most important, possibly transform the home aquarium industry from a threat, into a positive force for aquatic conservation.     Spin-gap opening accompanied by a strong magnetoelastic response in the S=1 magnetic dimer system Ba3BiRu2O9

    Neutron diffraction, magnetization, resistivity, and heat capacity measurements on the 6H-perovskite Ba3BiRu2O9 reveal simultaneous magnetic and structural dimerization driven by strong magnetoelastic coupling. An isostructural but strongly displacive first-order transition on cooling through T*=176 K is associated with a change in the nature of direct Ru-Ru bonds within Ru2O9 face-sharing octahedra. Above T*, Ba3BiRu2O9 is an S=1 magnetic dimer system with intradimer exchange interactions J0/kB=320 K and interdimer exchange interactions J'/kB=-160 K. Below T*, a spin-gapped state emerges with \Delta\approx220 K. Ab initio calculations confirm antiferromagnetic exchange within dimers, but the transition is not accompanied by long range-magnetic order.Comment: 5 pages, 5 figures, accepted by Physical Review

    Extension professionals are continually faced with the challenge of effectively communicating relevant information to an ever-evolving audience with diverse interests. Using focus group data, this article highlights specific educational programming needs of nonindustrial private forest landowners (NIPFs) in Mississippi. Findings indicate NIPFs are more likely to adopt new ideas if educational programming is tailored to their specific needs, consequently indicating the need to group the audience by their interests. Data also emphasize the importance of employing new technology as means for communicating more efficiently

    Objectives The purpose of this prospective study was to report incidence and transmission of SARS-CoV-2, among professional golfers and essential support staff undergoing risk assessment and enhanced risk reduction measures when considered a close contact as opposed to standard isolation while competing on the DP World Tour during the 2021 season. Methods This prospective cohort study included all players and essential support staff participating in 26 DP World Tour events from 18 April 2021 to 21 November 2021. High-risk contacts were isolated for 10 days. Moderate-risk contacts received education regarding enhanced medical surveillance, had daily rapid antigen testing for 5 days, with reverse transcriptase-polymerase chain reaction (RT-PCR) tesing on day 5, mandated mask use and access to outside space for work purposes only. Low-risk contacts typically received rapid antigen testing every 48 hours and RT-PCR testing on day 5. Results The total study cohort compromised 13 394 person-weeks of exposure. There were a total of 30 positive cases over the study period. Eleven contacts were stratified as 'high risk'. Two of these subsequently tested positive for SARS-CoV-2. There were 79 moderate-risk contact and 73 low-risk contacts. One moderate-risk contact subsequently tested positive for SARS-CoV-2 but did not transmit the virus. All other contacts, remained negative and asymptomatic to the end of the tournament week. Conclusions A risk assessment and risk reduction-based approach to contact tracing was safe in this professional golf event setting when Alpha and Delta were the predominant variants.     Mobile Open Systems Technologies For The Utilities Industries

    This chapter considers the provision of mobile computing support for field engineers in the electricity industry. Section 11.2 describes field engineers current working practices and from these derives a set of general requirements for a mobile computing environment to support utilities workers. A key requirement which is identified is the need for field engineers to access real-time multimedia information in the field and it is on this requirement that the remainder of the chapter focuses. Sections 11.3 and 11.4 present a survey of enabling technologies to support distributed systems operating in both local and wide area wireless environments. The impact of these technologies on the provision of mobile computing support is assessed in section 11.5. Section 11.6 describes a software architecture which attempts to address the requirements highlighted in section 11.2 and in particular is designed to support real-time access to data in the field. Finally, section 11.7 considers the degree to which utilities workers requirements can be met by the surveyed technologies and considers the likely impact of remote data access on field engineers working practices

    RNAseq reveals hydrophobins that are involved in the adaptation of aspergillus nidulans to lignocellulose

    Background Sugarcane is one of the world’s most profitable crops. Waste steam-exploded sugarcane bagasse (SEB) is a cheap, abundant, and renewable lignocellulosic feedstock for the next-generation biofuels. In nature, fungi seldom exist as planktonic cells, similar to those found in the nutrient-rich environment created within an industrial fermenter. Instead, fungi predominantly form biofilms that allow them to thrive in hostile environments. Results In turn, we adopted an RNA-sequencing approach to interrogate how the model fungus, Aspergillus nidulans, adapts to SEB, revealing the induction of carbon starvation responses and the lignocellulolytic machinery, in addition to morphological adaptations. Genetic analyses showed the importance of hydrophobins for growth on SEB. The major hydrophobin, RodA, was retained within the fungal biofilm on SEB fibres. The StuA transcription factor that regulates fungal morphology was up-regulated during growth on SEB and controlled hydrophobin gene induction. The absence of the RodA or DewC hydrophobins reduced biofilm formation. The loss of a RodA or a functional StuA reduced the retention of the hydrolytic enzymes within the vicinity of the fungus. Hence, hydrophobins promote biofilm formation on SEB, and may enhance lignocellulose utilisation via promoting a compact substrate-enzyme-fungus structure.
